Application Form. Transcripts are required from each post-secondary institution attended including PSEO coursework, even if courses are recorded on another transcript and regardless of the number of credits earned. A completed bachelor's or graduate degree from an institution that is accredited by one of the regional accrediting organizations - GPA of 3.0 or higher. A current state license or international equivalent as a Registered Occupational Therapist. Personal Statement. R sum . Two recommendations. Provide a copy of NBCOT certification or international equivalent. Applicants whose native language is not English are required to complete the TOEFL with a minimum score of 550 on the paper-based test or 80 on the iBT, or the IELTS with a minimum score of 6.5.
