  • 3. Front and back copy of Mother’s IC (Singaporean/ PR) or FIN card
  • 4. Front and back copy of Father’s IC (Singaporean/ PR) or FIN card
  • 5. Copy of the child’s colored passport-sized photo
  • 6. Copy of child’s passport details
  • 7. Child’s birth certificate (Singaporean only), Re-Entry Permit (PR only), or FIN card (copied on both sides)
  • 8. Child’s immunization records (Measles and Diphtheria are compulsory)
  • 9. Most recent school report/ portfolio from previous school or childcare setting
