NOTE: If the last school attended by the candidate was outside the UAE, School Report/transcript should also be attested by the following: 

a) Ministry of Education, Govt. of UAE 

b) Ministry of Foreign Affairs (in the country from which the student is relocating) 

c) UAE embassy (in the country from which the student is relocating) 

d) Consulate of the candidate’s previous country (in Dubai)

• A copy of the student’s passport and residence visa (wherever applicable) with the Unified Number (UID)
• A copy of the student’s Emirates ID
• A copy of the Father’s/Mother’s/Guardian’s valid passport and residence visa (wherever applicable)
• A copy of Father’s/Mother’s/Guardian’s Emirates ID
• A scanned copy of the student’s Birth Certificate
• Four recent passport-sized photos
• A copy of the Family Book (for UAE nationals only)
• A copy of the student’s Vaccination Card
• Student Medical File from previous school (including updated immunization records)
• Conduct Certificate/Recommendation Letter from the previous school
• Record of Attendance for the previous academic year from the previous school
• School Report/transcript from previous two academic years
• Specialist reports (for Students of Determination)
• Original Transfer Certificate from previous school (for students coming from another Emirate; attested by KHDA/Ministry of Education, Govt. of UAE)
