A letter from the parents explaining why they are changing schools (recommended for all other year levels; not applicable for prep or year 7 admissions). 
Recent reports from professionals (such as a paediatrician or psychologist, along with a signed statement of diagnosis) that were completed within the last 12 months. 
Parenting plan or Family Court Order (or Statutory Declaration if neither is available). 
Immunisation history documentation (downloaded via the MyGov app).
