Once conditionally accepted, students will be contacted through email by the Program Coordinator with information on how the specific program requirements will be met.Once you have been conditionally accepted to the Nursing Unit Clerk program, follow the steps below for the required program documentation:Health Assessment form must be completed by your doctor. Print a copy of the Health Assessment Record.Submit the form in person to Continuing Education at the Clearbrook Centre campus or by email to [email protected] must be up-to-date. Check with your high school, parents or Community Health Centre, as they could have a copy. Immunize BC provides tips for locating your immunization records. If you are not able to locate your record of immunizations, print the Immunization Record (NUC) form and have your doctor complete it. You will be asked for the record at the start of the program.Satisfactory Criminal Record Check and Vulnerable Sector Screening. This form will be completed in class at the start of the program.Health authorities require the immunization and criminal record check information before a student can be placed in a hospital for practicum.
