In a context of fragmentation of the healthcare system and faced with the requirement for continuity in monitoring the healthcare pathway of people with serious and/or chronic pathologies, the professional responsible for the coordination and integration of trajectories and systems ensures the management of interventions to meet the needs of a person or a group of people. The training combines university knowledge and professional know-how in the fields of health, medico-social and social. It brings together approaches from different backgrounds to reflect on innovative practices and organizations, open new interdisciplinary research fields and draw the beginnings of future health and medico-social systems.
