Completed application form. Submit Official Undergraduate transcripts in sealed envelope. Meet Kinesiology and Health Studies Departmental requirement of 2.75 GPA overall or a 2.75 GPA in the last 60 hours attemptedTwo recommendations. Statement of Goals. Submit a signed Technical Standards document. Pass a physical examination signed by a licensed medical physician. Provide Immunization records with HBV. Submit Professional Rescuer CPR and First Aid cert. or equivalent. Copy of passport. Statement of Financial Sponsorship. Proof of English Proficiency: TOEFL - 79 IBT or 550 PBT; IELTS - 6.5 overall band; Duolingo English Test - 105; Transcripts showing undergraduate degree was completed from an accredited university or college within the United States or from a UCO-approved native English-speaking country.
