Apply Online. Official Transcripts - Four-year degree and all prerequisites must be completed in the U.S. through a regionally accredited institution with a 3.0 cumulative GPA. Personal Information. Evaluations: (1) Academic and (2) Professional, preferred to be from a healthcare professional. Experiences: Qualifying Healthcare Experience and Volunteerism. Copy of Passport ID page. Declaration of Finance form. Proof of English Language Proficiency: TOEFL score of 80 or higher, IELTS score of 6.5 or higher, iTEP score of 3.9 or higher, Duolingo score of 110 or higher, PTE score of 58 or higher.
