d) Completed “Declaration Form” with a signature of the applicant
e) Completed “Letter  of Authorization for International Students” with the signature of the applicant
f) Two  letters of recommendation
g) TOEFL/IELTS scores if the applicant’s native language is not English.
h) TOEFL: iBT-54, CBT-157, IELTS: 4
